Privacy Policy

What this website collects, why it collects it, and who ever sees it. Written plainly, because you should be able to read it once and know where you stand.

Last updated: August 2026.

This policy describes how Harris Physical Therapy and Wellness currently handles information collected through this website. It is written for the website. If you are a patient and your question is about your protected health information specifically, please contact the clinic directly at (336) 214-5308 and we will answer it properly rather than pointing you at a web page.

Harris Physical Therapy and Wellness is a single-location practice in Hillsborough, North Carolina, with one clinician, Dr. Christian Harris, DPT, PT, OCS, CSCS. There is no marketing department behind this site. When you send something through a form here, it reaches the practice.

What this site collects, and why

Three things on this site collect information. Here is each one, exactly as it works.

The booking request form. It asks for your name, email address, phone number, the area of your body that hurts, roughly how long it has hurt, and what it is stopping you doing. It also has a checkbox where you can agree to receive text messages. We ask for the three pain questions so that the call back is a real conversation instead of an intake interview, and so that Dr. Harris knows before he dials whether this is something the practice can actually help with.

The benefits check form. It asks for your name, date of birth, ZIP code, insurance provider, member ID, an optional group number, and your phone and email. That information exists for one purpose: to look up what your plan says about physical therapy so you are not guessing. It is not used for marketing.

The Pain Check quiz. This one is anonymous. Your answers are stored in your own browser's local storage on your own device, not sent to us as you tap through it. If you then choose to start a booking request, those answers are carried into the form so you do not have to type them twice. If you never submit a form, we never see them. Clearing your browser data clears them.

Like almost every website, the host also records ordinary technical information when a page loads, such as an IP address, browser type, and which pages were viewed. That is used to keep the site up and to understand roughly how it is being used.

Text messages and SMS consent

The booking form has a separate checkbox for text messages. It is unticked until you tick it. Ticking it means you agree that the practice may text you about your request, scheduling, and appointment reminders.

  • Consent to texts is never a condition of treatment. You can leave the box unticked, submit the form, and be seen exactly the same way. We will call instead.
  • Reply STOP to any message to opt out. Reply HELP for help.
  • Message and data rates may apply, depending on your mobile plan.
  • Your phone number and your consent to be texted are never shared with anyone for their own marketing, and never sold.
  • You can also opt out by calling or emailing the clinic and asking. That works just as well as replying STOP.

How the information is used

  • To call or message you back about the request you sent.
  • To schedule, reschedule, or confirm an appointment or a free discovery call.
  • To check what your insurance plan says about physical therapy, if you asked us to.
  • To answer a question you sent through the site.
  • To keep the website working, secure, and free of spam submissions.
  • To meet legal, licensing, and record-keeping obligations that apply to a healthcare provider.

We do not use website form information to build advertising profiles, and we do not send marketing to people who only asked a question.

Who it is shared with

Your information is never sold. It is never traded, rented, or handed to a data broker.

It is shared only with the service providers the practice needs in order to run, and only so they can do that job:

  • Netlify, which hosts this website and processes form submissions before they reach us.
  • The practice's customer relationship management system, where submissions are forwarded so that requests are not lost in an inbox, and where scheduling and messaging happen.
  • The practice's email and phone providers, in the ordinary course of contacting you.
  • If you become a patient, the systems used for clinical records, billing, and scheduling, which are handled under the rules that apply to patient records rather than under this website policy.

We may also disclose information if the law requires it, for example a valid subpoena or court order, or where disclosure is needed to protect someone's safety.

Third-party content embedded in this site

Some parts of these pages are loaded from other companies' servers. When your browser loads them, that company can see the request and may set its own cookies, under its own privacy policy rather than ours:

  • A Google Maps frame on the location and contact areas of the site.
  • A Google Reviews widget that displays reviews the practice has received.
  • Google Fonts, which supply the typefaces the site is set in and are loaded from Google's servers.

We do not control what those services collect. Your browser settings and any tracker-blocking extension you use will affect them. Blocking them may stop the map or the reviews from displaying, but the rest of the site will still work, and you can always reach us by phone.

Cookies

The site itself uses only what it needs to function, plus the local storage described above for the Pain Check. The third-party embeds listed in the previous section may set cookies of their own. Most browsers let you block or delete cookies in their settings.

How long information is kept

Form submissions are kept for as long as they are useful for the reason you sent them, and then removed. In practice that means a request that never turns into an appointment is kept for a limited period so that we can follow up if you get back in touch, not indefinitely.

If you become a patient, your clinical record is a separate thing, and it is kept for the period that North Carolina law and professional licensing rules require for healthcare records. That is longer, and we cannot shorten it on request.

You can ask us to delete a website submission at any time. See the section on requests below.

A website form is not a medical record, and it is not a secure channel

This matters, so it gets its own heading.

Information you send through a form on this website is a business enquiry. It does not become a protected medical record until a patient relationship with the practice actually begins. Email and web forms travel over the ordinary internet, and no website form should be treated as a confidential clinical channel.

Please do not send detailed medical history, diagnoses, imaging results, medication lists, or anything else you would consider sensitive through this website. Tell us the area that hurts and how to reach you. Everything else belongs in the conversation, where it is handled under the rules that protect patient information.

And if this is an emergency, do not use a form at all. Call 911 or go to your nearest emergency department.

Children's privacy

This website is intended for adults. We do not knowingly collect information through this site from a child under 13. A parent or guardian is welcome to submit a request on behalf of their child, and when they do, we treat that information as the parent's submission. If you believe a child has sent us information through this website, contact us and we will delete it.

Your privacy rights

Depending on where you live, state law may give you specific rights over the personal information a business holds about you. California has such a law, and several other states have passed similar ones. In general terms, those rights include:

  • Asking what personal information we hold about you and where it came from.
  • Asking for a copy of it.
  • Asking us to correct something that is wrong.
  • Asking us to delete it, subject to records we are legally required to keep.
  • Opting out of the sale or sharing of personal information for advertising. We do not sell or share your information that way, so there is nothing here to opt out of, but the right exists and we will honor it.
  • Not being treated worse for exercising any of these rights. We will not refuse care, delay you, or charge you differently because you asked.

Rights that apply to a clinical record are handled separately, under the rules that govern patient records, and requests about those should come to the clinic directly.

How to request deletion or correction

Email admin@harrisphysicaltherapyandwellness.com or call (336) 214-5308 and say what you want done. Put "Privacy request" in the subject line if you email, and tell us the name, phone number, or email address you used on the form so we can find the right record.

We may need to confirm you are who you say you are before acting, especially on a deletion request. That is a protection for you, not an obstacle. We will respond in a reasonable time, and if we cannot delete something because a law requires us to keep it, we will tell you which category it falls into rather than going quiet.

Security

The site is served over an encrypted connection, and access to submissions is limited to the practice. No system is perfect, and we are not going to claim otherwise. What we can tell you is that the amount of information this website asks for is deliberately small, which is the most effective protection there is.

Changes to this policy

If how we handle information changes, this page changes with it and the "Last updated" date at the top moves. There is no separate notification list. The date is the honest signal.
